Auto injury claims are usually covered 100% by insurance.

You will need to have with you for your first visit:

1. Claim #

2. Date of Injury/Accident, insurance report of injury/accident

3. Attending Physician or Chiropractor, name, phone number, office address, Rx/prescription from doctor for massage therapy with Dx/diagnosis codes.

4. Insurance claim representatives name and claims mailing address

In office table and seated massage - The same as above

Out-Call/At Your Home Massage:

 60 minutes $70    90 minutes $100    120 minutes  $130

Additional Fee may be charged for locations over 25 miles from the office

Body Scrub in addition to massage:   $15

Aromatouch technique with DoTERRA essential oils 20 minutes add $15
