- You will need to bring the following with you at the time of your first visit:
- Claim#
- Date of Injury/Accident
- Insurance report of Injury/Accident
- Attending Physician or Chiropractor’s name, phone number, office address, Rx/prescription from doctor for Massage Therapy
- Attending Physician Dx/diagnosis codes.
- Insurance claim representatives name and claims mailing address
